Web Avatars: Erasmus and Kinkajou Case IV A Case of Chronic Fatigue & Fibromyalgia
DR Xxxxx: This patient gives a long history of chronic fatigue and sleep problems. Eventually Fibromyalgia like pains appeared. She had sore shoulders and sharp jabs in her chest at times, amongst other symptoms. She responded to Paill Spectrum treatment, but relapsed months after ceasing treatment. Paill Spectrum is a chronic relentlessly progressive illness. Many symptoms currently with no known cause, may have a Paill Spectrum cause.
This patient presents with chronic fatigue. Fibromyalgia develops later. This describes a "third wave" Paill Spectrum infection.
Many people complain of fatigue and tiredness. The aches can be very bad and so difficult to live with that people needs long term medications to restore their freedom from pain and to restore their energy levels. This pain is not arthritis. May people have sore shoulders or sore tendons : e.g. rotator cuff tendonitis as doctors would describe these symptoms and signs. It is often called Fibromyalgia and / or chronic fatigue. Other pains such as sharp chest jabs and chest wall pains feature in this patient's illness. These may not be isolated unrelated symptoms. Doctors can't tell you what is wrong. Maybe a virus. Blood tests for bad nutritional and food allergies may assist to choose appropriate treatments. Other medicines may help to relive symptoms and pain. Patients keep getting worse. Medicines often don't help, or only help for a while.

&run down.
Had head spins if she rolls over in bed at night.
No obvious head movement triggers the symptoms mentioned.
When examining her, moving the patient’s head does not trigger any head “spins”.
Presenting Complaint:
Breathless and short of breath on exertion if carries clothes off line, from lawn. Mentions she goes to sleep easily at 6.30 pm. >> ? chronic fatigue

Presenting Complaint
Vertigo (spins and wobbles)if lies down, rolls over, or gets out of bed.
Requests a painkiller for arthritis in knees and back.
Prochlorperazine (Stemetil) prescribed for positional vertigo, (wobbles and spins).
(This is a traditional medical treatment for this symptom).
Heart Echocardiogram and Chest X-ray are essentially normal when looked at today.

(Granuloma annulare is a knobbly rash at the elbow, in this patient . It is thought to be due to an immune reaction in the skin).

Thyroid tests
This is usually interpreted as being a normal thyroid , perhaps running little cooler than preferable
Complains of Pains in her left chest going through to her back including her left arm. There is pain in her shoulder and she finds it "hard to breathe."
The patient is very concerned that she may have pericarditis.
She also complains of having a wet cough with yellow -green phlegm and of having a fever.
Patient relates that she has had pericarditis before.
On Examination
No fever,
Pulse rate 84 regular
BP 160/100 (This is high).
Mildly red throat.
Investigations from recent hospital assessment are all normal
Treatment on this occasion: broad-spectrum penicillin antibiotics.

A gluten free diet was initiated.

Feeling better,
Bones are still sore in wrists, knees, neck, shoulder and base of spine.
Bloat gone,
Her nausea is gone, constipation gone, and wind (flatus) are gone.
Her energy levels are better and her tiredness shows some improvement.
Complains that if she works for a few hours, she cannot lift shoulders over 90 degrees from the vertical, due to the discomfort.
This discomfort takes a number of days of rest to settle.
Patient very unhappy and in a lot of pain.
Complains of pain so severe that its like the skin is crawling,
Complains of migraine like pain in her head.
Patient feels very bad in morning but warms up as she moves around.
Tramadol ( a supposedly non-narcotic painkiller) gives some pain relief only.
She describes some strange feelings in her head.
The impression is that these symptoms are a continuation of the preceding few months’ complaints.
The initial treatments of Paill Spectrum are specified on the treatments advice pages, but if used in children with the same clinical presentations, need to be modified for use by children in terms of both: doses of medications as well as types of medications.

: In interviews with our genesis doctor, the good doctor told Erasmus and Kinkajou about the years of consideration he has given the issue . His compatriot Dr Xxxxx says that after all the time spent learning to understand the advance inherent in the Paill Spectrum illness model, the theory works well and there has never been any experience or event that has cast any doubt about the theory and its practical applications. The medical establishment after looking at the issue for anything up to a few minutes believes the theory is wrong. The problem of course is that they have no idea what is right or what the answer is. Things just happen, seemingly by magic.
Erasmus: In the Paill Spectrum model of illnesses, there is NO event that occurs without a biomechanical cause. Everything happens for a reason. Often one of the Paill Spectrum reasons.
Kinkajou: So you reckon , there are no Acts of God.
Erasmus: Our doctor panel says "Every event that occurs in sickness or health", occurs as a result of changes in the functioning of the biological machinery of the body.
Kinkajou: How radical is that!
Erasmus: Hard to believe that in the 21st century , so many educated idiots believe in magic and only give lip service to the scientific method.

DR Xxxxx: Many doctors would not even agree that conditions such as Fibromyalgia or chronic fatigue even exist. Few would be able to describe the symptoms or signs of illness. Essentially none would recognise that neuropsychiatric symptoms form an integral part of the definition of chronic fatigue. What are neuropsychiatric symptoms? (I think the disease definition has moved on now, and excludes mental illness).
Dr AXxxxx : Fools! That single observation no mater how primitively seen or considered, is the chink in the armour of a truly incredible illness. Invisible but lethal. Fatigue is common in psychiatric illness. Fatigue is common in many other illnesses. It would never occur to any of these fools that the fatigue actually defines a real illness with possible real physical and mental symptoms.
One to rule them. One to find them. One to bring them in, and in the darkness bind them.
DR Xxxxx: I think that a number of the CDC criteria for chronic fatigue are wrong. An easy thing to say with hindsight, once you understand what you are up against.
Dr AXxxxx : An illness so aggressively pervasive, that people stop complaining as their attachment to the world of reason and the world of cause and effect becomes ever more tenuous. Then their memories disappear and the end is nigh.
Erasmus: So, tell us a story, doc. What have you seen?
