Kinkajou: How Might Paill Spectrum 
DR Xxxxx : There are several possible mechanisms by which Paill can affect bloods pressure.
Firstly, damage to nerve control mechanisms such as baroreceptors is possible.
Brain injury to parts of the brain responsible for homeostasis of blood pressure.
Renal mechanisms whereby adrenergic hormones from the action of the sympathetic nervous system divert blood from organs to the muscles. This action can result in prolonged vascular constriction of the glomerular arterioles of the kidney. When the afferent arteriole of the glomerulus is constricted for long periods of time, the blood flow deficiency affects the more sensitive parts of the glomerular / tubular structure.
There is another subgroup of patients who have puzzled me. There are a number of patients who inexplicably develop renal deterioration with rising creatinine (though often still within normal limits).

Perhaps the mechanism of injury relates to sympathetically mediated afferent arteriolar vasoconstriction reducing glomerular blood flow and reducing the flow of blood to the renal medulla, causing damage and loss of filtration units.
Not every patient has every Paill Spectrum symptom. Renal diseases are “capacity” diseases. Problems develop only when the capacity of the specific organ to function normally, is compromised. Patients with the most sympathetic activation (probably LIR variant Paill Spectrum patients), would be expected to develop the most severe forms of damage.(according to the last theory about damage).

DR Xxxxx : The Paill Spectrum model would predict that in early cases with hypertension, blood pressure could be stabilised with Paill Spectrum and related treatments. The situation for blood pressure is probably complex .

Alternately, if patients stayed on blood pressure medication, it may be expected that their blood pressure would become more responsive to therapy or easier to treat. Blood pressure fluctuations of instability would be predicted to be an early marker for the presence of Paill Spectrum renal damage. Many of these patients would over a number of years progress to develop frank hypertension.
A corollary of the mechanism of renal injury is that blood pressure forms a gradation of risk. People who are in the process of developing blood pressure would typically have a period of worsening blood pressure instability: so called white coat hypertension.
DR Xxxxx : The Paill Spectrum model would propose that Paill Spectrum may have a role in the genesis of hypertension through neurally mediated sympathetic mechanisms. The situation is likely more complex than this and bears further discussion at a later date. Back to Hypertension Top

Erasmus : Blood pressure or hypertension is a complex disease. Though the Paill Spectrum disease may have a role in the genesis of blood pressure, the situation is probably a lot more complex than simple damage caused by Paill Spectrum.
