Presbyopia
DR Xxxxx : Presbyopia: The standard explanation of this phenomenon in the commonly accepted medical model is that with increasing age, degeneration of the structure of the lens of the eye results in increasing stiffness and a reduction in plasticity of the lens of the eye. The internal eye muscles become less able to deform the lens of the eye. The ability to alter the shape of the lens is affected resulting in an inability to focus, usually at near distances, resulting in a need for reading glasses to enable focusing at close distances such as are typically encountered in "reading". it is unlikely that there is much evidence base for this opinion as research would demand the enucleation of apparently normal eyes to study" lens viscosity" and "pressure deformability".
Cataract surgery (a method whereby researchers may have access to fresh human lens material under normal circumstances), does not usually remove the lens and capsule and in removing the lens alters the lens structure, making any conclusions about lens deformability impossible.
Kinkajou: Presbyopia may affect different eyes at different speeds, to different extents and with varying levels of astigmatic change (either horizontal or vertical) in either eye. It does not appear to be a uniform process. Any better ideas Dr Xxxxx.

DR Xxxxx : In Paill Spectrum, the brain and nerve tissue are classically affected, but other body tissues such as muscles can be affected as well. Paill Spectrum could easily cause ocular damage by any of the neural mechanisms we have mentioned. Damage would also not necessarily occur when Paill spectrum disease is at its most active. Once the pathway of injury is created, even if the disease is stopped, the presbyopia can still progress in the disease models above. Paill Spectrum would easily be expected to affects the nerve dependent control of visual muscles and nerves associated with vision. It could however cause lens changes via effects on inflammation mediated by for example interleukin 2. Once the eyes lens is scarred or the lens cell linings controlling the internal ocular osmotic pressures are damaged, some degree of presbyopia would result.

Very different consequences in planning intervention or treatment, depending on the explanatory model being used. The stiff lens theory is unlikely to go away because it seems so intuitively correct. however the evidence base for this conclusion may well be very deficient .
The Paill Spectrum model is perhaps a better explanation for the development of symmetric and assymetrical presbyopic visual acquity deterioration, in the presence of nasal medical pathology.

To produce this result requires Paill to affect a specific part of a patient's sense of self at a specific time in puberty and will usually be more likely to occur in the presence of a positive male role model and a negative female role model. The "idea" can of course arrive at any stage of a person's life, but orientation occurs at puberty. The "idea" also will often reflect the predominant "environmental" preoccupation of the affected person.
